1. Who we are

This Privacy Policy explains how Moreira & Angeli LLC, doing business as Integrative & Functional Wellness Center ("IFWC," "we," "us," or "our"), located in Orlando, Florida, USA, collects, uses, and protects your information when you visit jifwc.com, fill out our forms or wellness questionnaires (including at public events), contact us, or receive our wellness services.

By using our website or providing your information, you agree to this Policy. If you do not agree, please do not submit your information.

2. Scope of this Policy

This Policy covers information we collect through our website, online and paper forms, wellness self-assessment questionnaires (such as a symptom/MSQ questionnaire), event sign-ups, and general communications with us. Additional terms may apply once you become a patient of our clinic (see Section 9).

3. Information we collect

We only collect what we need. Depending on how you interact with us, this may include:

  • Contact details you give us: first name, last name, email address, and phone number.
  • Wellness questionnaire responses: answers you provide in wellness or symptom self-assessments (for example, an MSQ or similar questionnaire). These are used to give you general educational wellness information.
  • Basic website/usage data: standard technical information such as browser type, device, approximate location, and pages viewed, collected through cookies or similar technologies to keep the site working and improve it.
  • Additional health information (patients only): if you choose to become a patient, we collect additional health information needed to provide care, as described in Section 9.

We do not knowingly collect more sensitive information than needed for the purpose you contacted us about.

4. How we use your information

We use your information to:

  • respond to you and follow up on your request;
  • schedule appointments and provide our wellness services;
  • send you the educational wellness information you asked for;
  • operate, secure, and improve our website and services.

Important: this is not medical advice. Our wellness self-assessments and general information are educational wellness tools only. They are not a medical diagnosis, medical advice, or a substitute for care from a licensed healthcare provider. Always consult a qualified professional about your health.

5. Why we contact you (consent)

We only contact people who have given us their information and agreed to be contacted. When you enter your email or phone number in one of our forms, you are asking us to reach out to you about your request. You can ask us to stop contacting you at any time (see Section 8).

6. Text messages (SMS)

We will only send you text messages if you separately opt in to receive them (for example, by checking an SMS consent box). SMS messaging is governed by its own terms, including message frequency, that message and data rates may apply, and how to opt out. See our SMS Terms at jifwc.com/sms-terms.

You can reply STOP at any time to unsubscribe from text messages, or reply HELP for help. Consent to receive text messages is never a condition of receiving our services. (Note: at public events we collect email only; SMS is a separate, future step and requires its own opt-in.)

7. How we share information

We do not sell your personal information, and we do not share it for advertising in exchange for money.

We share information only with trusted service providers who help us operate, and only as needed to provide our services. These currently include:

  • Twilio: to send messages and communications;
  • Supabase: database hosting;
  • Vercel: website hosting.

These providers are bound by agreements that limit how they may use your information, including a Business Associate Agreement (BAA) where applicable to protected health information.

We may also disclose information when required by law, to protect our rights or safety, or in connection with a business transfer.

8. Your choices and rights

You may:

  • access the personal information we hold about you;
  • correct information that is inaccurate;
  • delete your personal information;
  • opt out of our communications (email or SMS).

To make a request, email us at procurement@jifwc.com. We will respond within a reasonable time and as required by applicable law.

9. Health information and HIPAA

Information collected from the general public at an event or through a website form, before you become a patient, is not protected health information (PHI) under HIPAA.

When you become a patient of our clinic, the health information we collect to provide your care is handled in accordance with applicable law, including HIPAA where it applies. As a patient you receive a separate Notice of Privacy Practices that describes your rights regarding your health records.

10. Data security and retention

We use reasonable administrative, technical, and physical safeguards to protect your information. No method of transmission or storage is 100% secure, so we cannot guarantee absolute security.

We keep your information only as long as needed for the purposes described in this Policy, or as required by law, after which we delete or de-identify it. In general, we keep information from event leads and pre-patients for up to 24 months, or until you ask us to delete it. Patient health records are kept for as long as required by Florida law.

11. Minors

Our services and forms are intended for individuals 18 years of age or older, or minors with the consent and involvement of a parent or legal guardian. We do not knowingly collect information from children without appropriate consent.

12. Changes to this Policy

We may update this Policy from time to time. When we do, we will change the "Last updated" date above and, if the changes are significant, provide additional notice where appropriate. Your continued use of our services after an update means you accept the revised Policy.
